Journey to the heart of the metropolis at the 2016 Norwalk Hospital Gala: A New York State of Mind on Saturday, September 17, 2016 at the Hyatt Regency Greenwich in Greenwich, Conn.
Fine dining, dancing and Big Apple surprises await. Proceeds from this year's event will support Norwalk Hospital’s Community Care Team (CCT), a community-based outreach initiative that’s changing lives, saving lives and helping vulnerable Norwalk residents receive the care they need.
A Caring Community
Representatives from nearly 50 community and municipal agencies comprising the CCT work closely with a Norwalk Hospital-based CCT Navigator to improve outcomes for Norwalk residents suffering from mental health, substance abuse, and chronic medical conditions who are homeless and/or frequent utilizers of the Hospital’s emergency department.
The Norwalk CCT is making great strides in our community but needs additional resources to fully meet the needs of newly referred individuals.
