RVNAhealth Flu Clinic at Bethel Berry School on October 12

You are invited to get your flu shot from one of the nurses at RVNAhealth. The Flu Clinic will take place at Bethel Berry School, 200 Whittlesey Drive, on Wednesday, October 12 from 4:00 pm to 7:00 pm

To register visit RVNAhealth here (and read below):

1. Follow the link to select a date and time for your flu shot appointment.

2. Once you have selected a date and time, enter your contact information, and press green [continue] button

3. Review appointment details and consent. Check the box confirming that you read and agree to the consent, then press the green [Confirm Request] button.

4. IMPORTANT: Your appointment is pending until you complete an intake form. On the next screen, you will see a blue [Open Intake Form]. Please click this button.

5. You will be prompted to enter a password that you receive in your e-mail inbox. Check your ‘junk’ folder if you don’t see it. Enter the password and click the green [Open your form] button.

6. Complete all sections of the form. If you are able to upload an image of your insurance card, it will save time at the clinic.

7. Provide an e-signature and click the orange [Submit Form] button.

8. You will receive an e-mail confirmation of your appointment.
