Free Clinic for Community Members and BHS/BMS Students ages 12 and Up on Friday

On Friday, September 24, 2021, we will be hosting a COVID 19 vaccine clinic at Bethel Track and Field Center from 12 PM to 2 PM.  The second dose will be administered on October 15, 2021, from 12 to 2 PM at the same location.  This clinic has been coordinated through the Office of the Governor and the Connecticut Department of Public Health through a partnership with Griffin Hospital.  This clinic will be open to: 

  • Students ages 12 and up who wish to receive the Pfizer vaccine
  • Staff, parents, and/or community members who wish to receive the Pfizer, Moderna, or J & J vaccine
  • Anyone needing a second dose (Moderna or Pfizer).
  • Any individual who meets the criteria to receive a booster (immunocompromised with Doctor recommendation).  

Let us be clear, we will not vaccinate any students without written parental consent.   Although a parent does not have to be present for your child to receive the vaccine, we welcome any who wish to do so.   

If you would like your child to participate in this clinic, please complete this form. All written consent forms will be due by Wednesday, September 22, 2021, to your child’s health office or nurse.

Staff, family, and community members can walk in at any time during clinic hours to receive a vaccine.
