Putnam County DOH asks residents who test positive to self-Isolate and notify their contacts

Record breaking new cases of COVID-19 have exceeded the capacity for the Putnam County Department Of Health and the New York State Department Of Health to individually contact all persons who test positive or have been exposed to someone who has tested positive.

County residents who test positive are asked to self-isolate and to notify their contacts. Those who have been exposed to someone who has tested positive are being asked to self-quarantine.

Employers and schools may require County isolation or quarantine orders, accompanied by a self-attestation, which can be found at: https://www.putnamcountyny.com/health/covid19/#isolation.

A form to determine contacts, along with isolation and quarantine instructions, are also available at this website.
