Public Health Threat Influenza Vaccine Clinic
Due to the high incidence of flu in New York State, the Putnam County Department of Health is offering FREE seasonal flu vaccine to anyone who lives or works in New York State 6 months of age and older

Please bring proof of residency (driver’s license) and Medicare card (if applicable)
Parents with children from six months to three years of age should call the health department at 845-808-1332 to double check on current availability of the pediatric vaccine.
SAVE TIME! Vaccine consent forms are available on the Putnam County website at www.putnamcountyny.com/health/immunization. Download the forms, complete them and bring them with you to the clinic. Forms will be available at the clinic if you are unable to download them.
