Town of Brookfield Announces Vaccination Information

Getting a vaccine

Please note that some of you may be receiving duplicate alerts. More than one organization is notifying people of vaccination availability. This may create confusion but we are all working to get out this information to as many people as possible as quickly as possible.
 
The CDC-VAMS Email link went out from the town on Thursday to residents age 75 years and older who pre-registered their names and unique email addresses on the town of Brookfield website. If you do not see it in your inbox, please check your spam folder. Obtaining this link and filling in the information on the VAMS website is how an appointment is obtained for a vaccination.
 
The CDC-VAMS  has a Help Desk number 833-957-1100 if you need assistance. The town website also has step-by-step instructions for how to use the VAMS website.
 
If you are 75 or older and are not registered with the town, please complete this link:
https://brookfieldct.seamlessdocs.com/f/COVIDVaccine and we will reach out to you on Tuesday. You will need a unique email address for each person registering.

Please reach out to your elderly family member or neighbor who may not be comfortable with computer technology. Ask them if they need help with making an appointment for their vaccine. Again, using either access portal will require a unique email address for each person registering.
 
Seniors without internet access can call the Connecticut Covid Vaccine Appointment Assistance line at 877-918-2224.  We anticipate there may be wait times.
 
Brookfield will hold two clinics at the Brookfield Senior Center next Thursday and Friday. THERE ARE NO WALK INS OR PHONE APPOINTMENTS. We will be posting the Brookfield Clinics to the VAMS system today at 3PM. If you wish to obtain your vaccination at the town clinic, please get on the system today at 3PM to try and make an appointment. There are a limited number of VAMS appointments available at our clinics. The system is first come-first served and anyone in the State can book an appointment at any clinic that has available time slots.
 
The day before your appointment, you will receive a reminder and a medical screening survey in your email. You must complete this survey before arriving to your appointment. If this is not completed the staff will need to reschedule your appointment, per CDC guidelines. Please remember to dress properly so you can receive your vaccination in your upper arm.

As we receive new vaccine deliveries, we will schedule additional vaccination clinics. If you are over 75 and cannot find an appointment in Brookfield, you can schedule an appointment in another location. Keep checking our Town Website for up-to-date information.
