
The Carmel School District is pleased to be partnering with the Putnam County Health Department this fall for our Annual Influenza (flu) Clinic.
The Health Department will be on-site at each school building over the next several weeks offering flu vaccines for students and staff of the Carmel Central School District who sign up in advance.
Students will only be eligible to receive the flu vaccine if they submit the proper consent forms signed by their parent/guardian in advance of the clinic. Those forms should be returned to the main office in the building where the student attends school.
Building staff must also complete consent forms in advance of the clinic and return their forms to the main office in the school where they work.
Transportation Department staff should return their forms in advance of the clinic to the school where they will receive the vaccine.
District Office staff should return their form in advance of the clinic to Matthew Paterson Elementary School.
Consent forms and vaccine information statements we emailed directly to parents and staff, but can also be found on our Influenza Clinic webpage. Late forms will not be accepted.
The Department of Health will have only the number of vaccines for which they have received completed forms. Questions may be directed to the Putnam County Department of Health at (845) 808-1332 x43113.
