The Watermark at 3030 Park Announces Provider Partnership with Yale New Haven Health Post-Acute Network

Bridgeport, CT - The Watermark at 3030 Park is excited to announce the retirement community’s rehabilitation and skilled nursing neighborhood, The Springs, has been chosen to be a provider to the Yale New Haven Health post-acute network which includes neighboring Bridgeport Hospital. This new designation positively affects all present residents of The Watermark as well as seniors living within the broader community.

Yale New Haven Health’s development of the post-acute network strengthens the hospital’s post-acute connection by improving patient outcomes and decreasing length of stay and readmissions. In order to be admitted into the Yale New Haven Health post-acute network, The Watermark at 3030 Park has and will continue to fulfill the following requirements:

• Maintain a quality rating of 3 stars or higher on Medicare Nursing Home Compare site (Springs at The Watermark has a long standing five star rating)

• Work with Yale New Haven Health Systems (YNHHS) to develop and utilize clinical pathways and protocols for conditions such as Congested Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), Pneumonia (PN) and hip fractures

• Exhibit outstanding performance in the following areas:
  •    Hospital readmission during a skilled nursing facility episode of care
  •    Skilled nursing facility length of stay
  •    Department of Public Health care management satisfaction survey

• Participate in YNHHS initiatives designed to improve hospital transitions that reduce readmission and transform care for each patient

• Deliver coordinated high-quality care to Medicare patients across the continuum of care which includes all levels and intensity of care

• Ensure the patients get the right care at the right time while avoiding unnecessary duplication of services and reducing medical errors
