The New Canaan Health Department is asking for help from residents.
We have collaborated with Norwalk Hospital and the Western Connecticut Health Network to conduct a New Canaan health needs assessment. This information is vital to the development of our Community Health Improvement Plan (CHIP). As part of this process the Western Connecticut Health Network is conducting a short community survey to learn more about you and our community's health needs. Your voice matters to this process.
