Booking Your PCOS Appointment
In order to get you started, I have outlined the basic steps to booking an initial PCOS consult:

Obtain referral – We want all patients to have referrals from an outside physician (pediatrician, gynecologist, or any other physician). The reason for this is because often times, certain gynecological and endocrine issues can be easily addressed by a GYN or primary care provider. However, if your Doctor believes you present with PCOS symptoms and would benefit from seeing a PCOS specialist, they can then refer you to us.

Have patient’s demographics and insurance information available.

Schedule initial evaluation with Diana D’Amelio, RPA-C & Head of our adolescent PCOS program.

Schedule initial nutrition consultation with one of our Nutritionists.

Complete new patient paperwork and submit relevant medical records at least two days before the initial consultation.
