The Ridgefield Police Department has updated its Special Needs/Disabilities Program. Participation is voluntary and authorizes the disclosure to, and use of, your medical information by first responders for the purpose of offering assistance when involved in an incident. This program acts as a facilitator only.
This medical form is designed to supply first responders with critical information about you during an emergency when you might not be able to communicate yourself.
All information provided on this form is confidential and it is the individual's responsibility to update as needed.
